Application

Admission Criteria:

  • You are 16 years of age or older.
  • You are seeking Recovery from the Disease of Addiction (Substance Use Disorder).
  • Physical and Psychological health is adequate for participation in the program.
  • Clients must be able to care for their own needs.
  • Willing to attend recovery fellowship meetings such as A.A. or N.A.
  • Able to make a 90-Day (3 months) commitment.
  • Must participate in all scheduled group therapy, individual counselling sessions, and all assigned activities.
  • You are motivated to make positive lifestyle changes and to work towards complete abstinence from all mood-altering substances to achieve long-term, sustainable Recovery.
  • Opiate substitution therapy accepted (Methadone, Suboxone, Sublocade).

2) Initial Contact, Screening, and Telephone Intake Assessment
